Donald Trump's former physician -- who rushed to his side after the former president was wounded last week -- has just provided an update on Trump's condition.

Congressman Ronny Jackson -- who served as Physician to the President under both Trump and Barack Obama -- put out a memo Saturday detailing Trump's injury in the aftermath of last Saturday's assassination attempt.

In the memo, Jackson says he rushed to meet Trump in Bedminster, New Jersey hours after the shooting took place, and he's stayed with him since, attending to his care.

Donald Trump's former COVID czar Anthony Fauci doesn't think very much of the Republican presidential nominee's bullet wound that he suffered during his attempted assassination.

In fact, Fauci called the injury to Trump's right ear "superficial" during a televised interview Friday with CNN host Wolf Blitzer.

In the document obtained by TMZ ... Richard's manner of death is not listed -- instead, it notes the manner of death is still pending an investigation. This explains why there is still no cause of death to report.

However, it appears the fitness guru -- who is actually referred to as a "Health Motivator" on the cert -- has already been laid to rest a week after his death. Per the doc, Richard was buried here in L.A. at the Pierce Brothers Westwood Village Memorial Park and Mortuary.

Richard Simmons' housekeeper tried to get him to a doctor after he fell in his bathroom, but Richard pushed it off, because he just wanted to stay home for his birthday ... TMZ has learned.

As we reported, Richard fell Friday evening at home, in a bathroom ... and according to law enforcement sources, when his housekeeper came to help him up, she implored Richard to seek medical attention that night.

We're told Richard had described feeling dizzy before the fall, but he rejected the housekeeper's suggestion about seeing a doctor ... telling her maybe he'd go Saturday, but particularly didn't want to go that night as it had been his 76th birthday. So, she helped him back to bed, and that was that.
