Scott Disick Plans to Sue Rehab Over Leaked Photo ... Facility 'Sickened' by Privacy Breach

9:44 AM PT -- 5/5 -- Noah Nordheimer, President & CEO of All Points North Lodge, tells TMZ ... his team is aware of the Disick photo and says, "The protection of our clients' personal and confidential information is of the utmost importance to us."

Nordheimer says it's strict APN policy to not disclose any personal info on patients, including whether or not a client is being treated there, and if it's determined that "any information relating to any client was ever obtained from APN facilities and provided to a media outlet" ... it will take legal action against that person.

He says the APN Lodge team is "sickened by any publication who would report on an individual’s life struggles without their consent.”

Scott Disick is enraged he's been betrayed by the rehab facility he's been staying at ... he's checked himself out and is going to sue.

Scott has been staying at the All Points North Lodge in Edwards, Colorado since last Tuesday. There are reports he's there for cocaine and alcohol abuse ... he's struggled with substance abuse in the past.

His lawyer, Marty Singer, says, well a lot. For starters, Singer says Disick was NOT there for coke and booze. He says, "In an effort to finally come to terms and deal with the pain that Scott has been silently suffering for many years due to the sudden death of his mother, followed by the death of his father 3 months later, Scott made the decision to check himself into a rehab facility last week to work on his past traumas."

We're told he was quarantining in L.A. by himself and it gave him time to realize he had not properly dealt with the death of his parents.

As for Scott's photo that was taken inside the rehab facility ... Singer says it's an outrageous HIPAA violation.

Scott's team believes the photo was taken by a staff member and not a patient, and that could trigger criminal prosecution. We're told the photo was taken of Scott upon his check-in to the facility.

As soon as he learned of the photo leak, Scott immediately called for a car to pick him up, and he's now left the facility and is en route back to L.A.

Though Disick and Co. maintain this has nothing to do with drugs or alcohol, he has struggled over the years -- in and out of rehab at least 3 times ... not to mention at least one hospitalization as a result of substance use. It wasn't until he started dating Sofia Richie in 2017 that he cleaned up his act.

Jim Montgomery Fired Dallas Stars Coach I'm Going to Alcohol Rehab

NHL coach Jim Montgomery -- who was recently fired by the Dallas Stars -- says he's admitting himself into rehab after the "wake-up call" of losing his job.

50-year-old Montgomery got the ax in December over a mysterious incident that nobody seems to be talking about. But, whatever happened was so bad, the pro hockey team fired him with cause and voided the remaining 2.5 years on his contract.

Now, Montgomery is breaking his silence ... and while he's not explaining the actions that caused his termination, he claims he's had a break-through moment and realizes he needs help.

"Losing my job as head coach of the Dallas Stars last month was a wake-up call," Montgomery said ... "It was also the appropriate call."

"I let the team’s front office, staff and players down. More importantly, I let my wife and my family down."

"The team’s decision to end my role forced me to look into the mirror and decide whether I wanted to continue living a damaging lifestyle or get help. I decided to get help. I turned to professionals in the field of alcohol abuse for their guidance and counseling. It has been an overwhelming and a very humbling experience knowing that I am not alone."

He continued, "Today, with the unconditional support of my wife and family, and many close friends, I took another step forward by admitting myself into an inpatient residential program, where I intend to take the steps to be a better husband, father, friend, coach and mentor – one day at a time."

"It’s a process I am committed to. As I do this, I ask that my family’s privacy be respected. Thanks, Monty."

Dallas Stars general manager Jim Nill also commented -- saying, "We are supportive of this decision by Jim and we hope that by pursuing this help, he and his family will be stronger for it."

Pete Davidson took many people aback on 'SNL' by seemingly announcing he was going to rehab for the holidays -- and if he's serious ... Alonzo Bodden has some advice.

We ran into the comic Sunday at LAX and asked him to weigh in on what appeared to be Pete implying he'd be checking into a rehab facility soon -- something he danced around on 'Weekend Update' without saying "rehab." It couldn't have been clearer though.

Alonzo -- who's a recovering alcoholic -- tells us he doesn't know Pete, but acknowledges there's usually more going on behind a person's public persona.

That said, he explains how rehab is different for everybody -- as no one goes in for just one thing. So long as whoever checks in gets the specific treatment they need for whatever their issue is, that's all that matters. If Pete falls into that category, AB wishes him the best.

As for whether going public with the potential rehab stint is a good idea, Alonzo's got some thoughts there as well. He says it's a double-edged sword, which can pay off big time.

In Pete's case, it certainly did ... he got an outpouring of love and support from 'SNL' fans online, who commended him for being so forthcoming with his demons.

Kodak Black I Need Drug Rehab While in Prison!!! Shorter Sentence Would Be Nice Too

Kodak Black wants approval to enter a drug rehab program while he's serving his Florida prison sentence -- but he could end up getting more than just clean and sober.

According to docs filed Monday in federal court, the rapper is asking the court to send him to a prison that offers the Residential Drug Abuse Program. The 500-hour program lasts 9 months and involves group therapy, as well as one-on-one treatment.

It also requires all participants to live in a separate unit from the general population -- a pretty clear benefit to a celebrity rapper doing time.

The biggest fringe benefit, though, for Kodak could be shortening his sentence. Upon completion of the program, inmates can get as much as 12 months knocked off their prison bid.

It's an interesting request from Kodak, who -- as we first reported -- claimed last week, someone in his current prison had drugged him, causing him to get involved last month in a prison brawl.

He's serving 46 months right now on federal weapons charges, and still has to deal with a sexual assault case in South Carolina.

The judge still has to approve his rehab request.

Ben Affleck is trying to get past falling off the wagon by moving forward with work ... even if he looks a little worse for wear.

Ben was spotted Tuesday with a script in hand in New Orleans as he prepared to begin shooting his new film, "Deep Water." Unclear if Ben's disheveled look -- messy hair, scraggly beard, wearing sweats and a stained t-shirt -- was part of his role. It's worth noting ... he's supposed to play Vic Van Allen, an attractive husband.

Bam Margera says he got some very special visitors at the rehab facility where he's getting treatment -- namely, a show of support from Steve-O and Johnny Knoxville!!!

We got Bam leaving Antonio's on Melrose Friday, and he said he'd just finished having lunch with one of his rehab sponsors. Don't worry ... we're told he washed down his Mexican food with a Sprite.

Bam told our guy treatment is going great, thanks in part, to all the support he's getting from his "Jackass" costars. He says they recently paid him a visit down in Orange County.

It's good to see Bam looking healthy ... as we reported, he landed in the hospital shortly after beginning his third stint this month in rehab -- but he assured fans he would be just fine. Bam said he had edema, swelling in the body caused by blood vessels leaking fluid into tissue.

Bam did tell us he's totally down to make more 'Jackass' movies once he's healthy again -- but he says there is one slight holdup outside of his issues.

The "Jackass" star was hospitalized Wednesday to treat what he says is edema ... swelling in the body caused by blood vessels leaking fluid into the tissue. Bam wrote in a since-deleted Instagram post that he was in the hospital for only a few hours to treat "either reaction to new medicine or detoxing from i guess a bit of alchohol [sic]." Bam Margera is putting himself back on the road to recovery -- on the heels of leaving rehab yet again, and ending up in a bar ... we've learned he's about to give rehab another shot.

Sources close to Bam tell TMZ ... he's voluntarily entering a new rehab facility on Thursday, knowing full well he needs help. We're told he intends to try to stay sober.

He was at a Los Angeles bar Wednesday night sipping on an ice cold mug of beer. He had just recently bailed on a treatment facility -- the second time he's done so this month.

So now, he's making attempt number 3, and we're told Bam's big stumbling block in treatment has been wanting his cell phone with him at all times, so he can communicate with his son. The last facility took his phone, which he didn't take well.

Our sources say Bam's checking into a different facility this time, and he's hoping they'll let him keep his phone.

Bam Margera was readmitted into a rehab facility just hours after his bizarre arrest at an L.A. hotel -- TMZ has learned -- and this time around he'll have to follow some serious guidelines.

Sources close to Bam tell us ... immediately after he was released from police custody, he was readmitted to rehab. The facility is demanding a commitment from him, along with various rules he must follow during his treatment. We're told he's agreed to all conditions.

Bam was first admitted to rehab last week after he publicly reached out to Dr. Phil and begged for help. The two did a sit-down interview and he was supposed to enter a 60-90 day program ... but he lasted less than a week.

TMZ broke the story ... Bam was arrested in L.A. early Wednesday morning at a Sunset Strip hotel. Cops say they were called to the hotel after Bam got into it with other hotel guests, and refused to leave the property.

Bam had been spinning a weird tale for bar patrons -- that he'd been hired by one of their wives to catch someone at the bar cheating on her.

We've spoken to family sources who tell us they're worried sick over Bam's health and safety ... telling us they believe he's off his bipolar meds, which is exacerbating his bizarre behavior.

Charlie was out Thursday in Bev Hills when a photog asked him about Brooke's latest battle with addiction, and as it turns out -- he'd just gotten off a phone call about that very subject. Charlie said someone had told him about his ex-wife reportedly checking into a trauma center to seek professional help for substance abuse.

The news comes after a years-old video started circulating reportedly showing a woman identified as Brooke doing crystal meth. She reportedly also tried to score drugs last month in the Hamptons.

Charlie -- who has 10-year-old twin boys with Brooke -- said the kids are doing great. The boys, Bob and Max, are reportedly staying with Brooke's parents while she's in rehab.

Sounds like Charlie, who now looks like the picture of good health, hasn't spoken directly to Brooke.

However, he says if she reaches out he'll gladly lend a helping hand.

HBB begged her mom to straighten out her life a while back, tearfully pointing out that she's currently living with her older sister, Pumpkin ... and not by choice but by necessity. It's pretty obvious she wants to be with her mom, but not in a chaotic, unstable and dangerous environment.

And, there's this ... we're told if push comes to shove and nothing changes, Pumpkin will go to court and fight to gain custody of her little sister. Our sources say so far, June hasn't expressed interest in getting her daughter back.

Kit Harington Making Time for Fitness ... During 'Wellness' Stint

Kit Harington is finding healthy ways to release stress -- getting in a workout down the street from his stay at a "wellness center" ... aka rehab.

The "Game of Thrones" star was walking out of a gym Friday in the Connecticut neighborhood where he's been taking what his people call a mental health retreat ... at a luxury facility.

Kit, who played Jon snow on the extremely popular HBO series, was rocking jeans and a button down on his way out of the gym. His female companion appears to be someone connected to the facility -- maybe a sponsor ... she's been seen with him around town in late April.

As we reported ... Kit's reportedly getting treatment for exhaustion, stress and alcohol abuse at a facility that's believed to cost about $120,000 per month. As you can see, he's allowed to leave the facility from time to time.

We're told Kit spent about 90 minutes at the gym ... plenty of time to get in a killer workout.

Kit Harington Rep Says NOT in Rehab ... It's a 'Wellness Retreat'

3:34 PM PT -- Kit's reps got back to us, and say he "has decided to utilize this break in his schedule as an opportunity to spend some time at a wellness retreat to work on some personal issues."

They say he's NOT there for "rehab," and emphasize the words, "wellness retreat."

Kit Harington was hit hard by "Game of Thrones" coming to an end -- so hard, he checked himself into a rehab center for stress and alcohol before the final episode aired.

The actor, who famously played Jon Snow on 'Thrones,' entered a luxury Connecticut facility for a mental health retreat weeks before the May 19 finale ... according to the NY Post, which first reported his rehab stay.

Kit's been getting treatment for stress, exhaustion and alcohol abuse. The Post says his wife -- fellow 'GoT' star Rose Leslie -- has been very supportive of the decision to get professional help.

The rehab facility is believed to cost over $120,000 per month ... no word yet on when Harington will be released.

As you know, 'GoT' wrapped its highly-anticipated 8th final season earlier this month. Reviews of the whole season were extremely mixed, at best, and downright awful at worst. It still got HUGE ratings, though.

Video of Kit reacting to the final script in the 'GoT' behind-the-scenes documentary as he read the fate of his character and Emilia Clarke's Daenerys Targaryen went viral ... because he broke down in tears.

Clearly, he wasn't just being dramatic. He has since said he needed therapy after learning how the show ends for his character.

Kit's also been very open about how much the show's worldwide attention has impacted his life. He told Variety the "darkest" point was when Jon Snow was killed ... but later came back to life.

Paris Jackson is in a much better head space since her suicide scare ... spending time with friends, family and taking advice from people who genuinely care about her.

TMZ broke the story ... Jackson was rushed to a Los Angeles hospital last week after she attempted suicide by cutting her wrists. She was released from the hospital a few hours later after allegedly telling doctors she'd seek treatment at another facility ... but that never happened.

Those close to Paris had originally pushed rehab on the 20-year-old -- but she refused -- and has taken her well-being into her own hands.

Family sources tell us Paris is trying to improve her mental and physical health, and open to listening to the folks she trusts. Our sources say Paris is eating better, exercising and working on psychological issues.

Paris will turn 21 in just over a week, and organizing the party has kept her busy. We're told Paris has also been in regular communication with her younger brothers ... and is proud Blanket is gearing up for college. Macaulay Culkin -- who had an unbreakable bond with Michael Jackson -- has also been a big support.

Wendy Williams just made a very personal revelation -- she's now getting round-the-clock help to deal with addiction issues.

She made a very emotional announcement Tuesday on her talk show ... revealing she's currently living in a sober house somewhere in the Tri-State Area -- of NY, NJ & CT -- and has been for some time now. Wendy teared up quite a bit as her audience listened intently.

Wendy talked about battling cocaine addiction in the past, but never sought treatment to stay clean ... until now, that is. She didn't specifically say what substance she's addicted to now. Wendy added she has a 24-hour sobriety coach, and spoke lovingly about the "brothers and sisters" in addiction with whom she's living now.

She took an extended leave from her show back in mid-January and didn't return until a couple of weeks ago. As you know ... she also told viewers last year she was suffering from Graves' disease before taking a 3-week hiatus.
