Mama June needs help -- and her family recognized as much when they confronted her in an intervention, which features MJ in a decrepit state ... busted teeth and all.

June was sat down by a roomful of family members earlier this month in a staged intervention that went down at her sister's home. With the help of Dr. Ish Major, her family pled with her to straighten out her life ... which appears to mostly be falling on deaf ears here.

One of the more heartbreaking moments ... June's youngest daughter, Alana -- aka Honey Boo Boo -- breaks down crying at one point, saying she wants to move back home.

Check it out ... Mama June has become a shell of herself since she got arrested for possession of crack cocaine in March with boyfriend Geno Doak. Her teeth -- which she'd recently gotten fixed up -- are literally falling apart. She also collapses on camera.

We've also obtained a clip -- that will air this week -- of June's daughter, Pumpkin, and her sister discussing June's arrest from earlier this year.

Rather than stay away from Geno -- like her family has suggested, and a judge ordered -- she's been shacking up with him since the arrest ... and has been hitting the slots on and off a lot as well. BTW ... we're told Geno wasn't present for the intervention.

A second woman just testified that ex-NFL star Kellen Winslow Jr. threatened to murder her while raping her in 2018 ... echoing allegations from another woman who testified Tuesday.

The 58-year-old woman -- identified as Jane Doe #2 in court -- alleges Winslow picked her up on May 18, 2018 in the San Diego area in order to take her on a coffee date.

But, instead of getting drinks at a nearby cafe ... she says Winslow pulled over on the side of the road in a remote area and raped her.

She claims during the act, he put his hands around her neck and said, "'If you scream,' I don't know if it was, 'I'll choke you to death' or 'I'll murder you.'"

The woman says after Kellen was finished ... he drove her away from the area and again threatened to kill her "if you tell anyone."

The woman says she thought she was going to die multiple times during the encounter, telling the court Wednesday, "I was very afraid. The whole time I thought I was never going to get out alive."

Remember, on Tuesday, another woman using the alias Jane Doe #1 told a similar story -- claiming Winslow picked her up while hitchhiking in 2018 and violently raped her before threatening to kill her if she told anyone.

Winslow has been in court all week facing his accusers -- he's been charged with multiple felonies including kidnapping and forcible rape. If convicted on all counts, he faces life in prison.

For his part, Kellen has denied all of the allegations.

Michael Avenatti Indicted Again ... Allegedly Stole $$$ from Stormy Daniels

Michael Avenatti has just been indicted on new charges ... alleging he stole money from a former client ... and sources tell TMZ it's Stormy Daniels.

The U.S. Attorney for the Southern District of New York has charged the disgraced attorney with 1 count of wire fraud and 1 count of aggravated identity theft charges. The feds say he fraudulently diverted money the client -- Stormy -- was owed on a book deal to an account he controlled.

According to prosecutors, he did this by sending a fraudulent letter purporting to contain his client's signature to Stormy's literary agent. According to the complaint ... Stormy didn't know the letter existed much less sign off on it.

He allegedly used the money from that account to pay for airfare, hotels, restaurants, meal deliveries and meet payroll for his businesses. In total, he allegedly diverted around $300k ... and although he sent her around half that, prosecutors say he hasn't paid her the balance.

Avenatti just denied the charges, tweeting, "At no time was any money misappropriated or mishandled." He went on to explain how he was entitled to some of the money as part of his fee.

If convicted on the new charges, Avenatti faces up to 20 years for the wire fraud count and a mandatory term of 2 years on the aggravated identity theft charge.

In addition, Avenatti's also in the feds' crosshairs for allegedly trying to extort Nike outta more than $20 mil.

Meek Mill D.A. Wants Him to Get New Trial ... In Front of New Judge!!!

Meek Mill's been getting a raw deal from Judge Genece Brinkley and deserves a new trial without her on the bench ... and that's according to the District Attorney in Philly.

The D.A.'s Office just filed new docs in Pennsylvania Superior Court stating Judge Brinkley's appearance of bias has been evident throughout the case ... and therefore, Meek should be retried in his probation violation case.

You'll recall, Judge Brinkley sentenced Meek to 2 to 4 years in state prison -- which was eventually set aside when Meek was released in April 2018 ... promptly boarding a chopper with 76ers owner Michael Rubin.

Since then, he appealed and asked for a new trial, but Judge Brinkley shot him down last June ... refusing to grant the retrial.

It's kind of extraordinary ... with Wednesday's filing, you now have the D.A. on the record siding with the defendant. In the docs, obtained by TMZ, the D.A. calls out Brinkley's bias against Meek ... singling out a surprise visit she made to a homeless shelter where Meek was doing court-ordered community service.

The D.A. points out that's not what judges do ... that's for probation officers who report back to the judge. According to the docs, Brinkley also "abused her discretion" when she hit him with the 2 to 4 year sentence for "technical violations" of his probation. Still, she has repeatedly refused to recuse herself from the case.

Even the D.A. says Meek serves no threat to the community, and had completed drug treatment -- and, therefore, deserves another shot in court ... sans Brinkley.

Kodak Black Begs Judge, Let Me Go to S. Carolina ... I Got a Case There Too

Kodak Black has criminal cases to deal with in multiple states, and his latest arrest in Miami is complicating things, big time ... so he's begging the judge for an assist.

As we've reported ... the rapper was arrested 11 days ago during the Rolling Loud festival, and though he was granted a pre-trial release -- if he can post a $500k bond -- the feds are appealing that and want to keep him behind bars.

Kodak's problem is he's also facing a serious felony charge in South Carolina for sexual misconduct, and needs to appear in court to stand trial. So, he's proposing a compromise to the federal judge in Florida ... let me out, but keep me under 24-hour surveillance while I'm traveling.

In new docs, the rapper says he still wants out on bond, and proposes a system of nonstop, round-the-clock supervision to ensure he makes all of his required court appearances ... and then gets back to Florida.

As we've told you ... federal prosecutors claim Kodak is too risky for bail due to his criminal history and want to him behind bars, but his legal team claims the feds don't have evidence to support that.

Fatboy SSE Busted for Weed After Traffic Stop in NJ

Fatboy SSE -- the comedic social media influencer co-starring with Master P in his "I Got the Hook Up" sequel -- hooked up with cops who busted him for marijuana and outstanding warrants.

According to cops in Jersey City, Fatboy was pulled over Monday night for a traffic violation. He was in the car with 2 other guys -- his manager and security guard -- and when cops ran his license, they discovered it was suspended. First mistake.

Second mistake? They found outstanding bench warrants ... which prompted a search of his car ... which is when cops say they found the weed.

Cops arrested Fatboy -- government name Tyriq Kimbrough -- for 1 count of possession of weed and bail jumping for the 2 bench warrants. Adding insult to injury, cops also slapped him with tickets for failure to observe a signal and driving on the suspended license.

Fatboy -- who has more than 5.5 million Instagram followers -- remains in custody.

His management posted a message on his IG page about the arrest, saying ... "If you have not been able to reach him this is why."

Master P signed the comedic actor -- known for his hilarious IG skits -- for a role in "I Got the Hook Up 2" ... which comes out this summer. We're gonna recommend a limo, or at least Uber, for Fatboy's ride to the premiere.

Ex-'Bachelor' Chris Soules Sentencing Delayed ... in Fatal Car Crash Case

12:37 PM PT -- The 2-plus year legal drama will continue as Soules' sentencing has been delayed. It'll be rescheduled for a later date. Defense attorney Robert Montgomery made a last-ditch effort to get the court to throw out the victim impact statements ... and the judge agreed to grant the motion.

Because the victim impact statements were thrown out, the judge also granted a motion to draft a new pre-sentence investigation report ... requested by both the defense and prosecutors.

Former 'Bachelor' Chris Soules will finally learn his fate in his fatal car crash case when he arrives to court for his sentencing at 11:30 AM PT ... and TMZ will be streaming live.

Soules is expected to arrive in Buchanan County, Iowa court any moment now and finally put the 2-year-old case behind him when a judge hands down his sentencing. As we reported ... Soules struck a plea deal back in November 2018 ... pleading guilty to 1 count of leaving the scene of a personal injury accident, which is an aggravated misdemeanor.

It was a HUGE plea deal considering he initially pled not guilty to a felony charge of leaving the scene of an accident resulting in death. As we first reported ... Soules was behind the wheel of a Chevy pickup when it rear-ended a John Deere tractor, sending it into a ditch.

The driver was taken by ambulance to a hospital, where he died. Chris was the one who called 911 to report the accident but prosecutors said he fled the scene before cops got there. Soules was later arrested at his house, but not before refusing to come out at first.

Soules, who was "The Bachelor" for season 19 in 2015, could get up to 2 years in jail.

Netflix's 'Jailbirds' Megan 'Monster' Hawkins Arrested ... Back in Sac County Jail

One of the female inmates featured in Netflix's new docuseries about life on the inside is right back where she started on the show ... behind bars in good old Sacramento.

Megan Hawkins -- known as "Monster" on the series -- was arrested in Elk Grove, CA Friday after she allegedly tried opening a bank account with an ID with someone else's name on it. Cops say that one of the bank employees recognized her from the show and called it in.

According to Elk Grove PD ... Megan was gone by the time officers arrived, but she hadn't gotten far. Police say they found her parked nearby in a vehicle that'd been reported stolen ... with tons of credit cards that didn't belong to her and a controlled substance.

She was arrested and booked for five felonies -- ranging from identity theft to forging or altering checks, just to name a couple. She's being held on a $10,000 bond in Sacramento County Jail -- the same place where she's featured for Netflix's new hit.

Megan is one of the most recognizable characters on the docuseries -- her tongue is split right down the middle ... hence the whole "monster" thing.

This photo is fitting for the nickname too -- it's one heck of a beast-y mug shot, if you ask us. Stay classy, Sac.

Chad Kelly Signs with Colts After Working with Uncle Jim Kelly

Troubled QB Chad Kelly just signed a new contract with the Indianapolis Colts -- and it begs the question ... is his uncle, Jim Kelly, to thank?

Here's the deal ... Jim had been working out with Chad this offseason to help him get a second shot in the NFL after he was cut by the Denver Broncos in 2018 following a bizarre arrest.

But, it's also hard to ignore the Jim Kelly connection with the Colts -- a team coached by Jim's former backup, Frank Reich!

Remember, Frank famously took over for an injured Jim Kelly in Buffalo during the 1992 NFL playoffs and led the Bills to the most historic comeback win ever when they were down 35-3 at halftime to the Houston Oilers.

So, did Jim call in a favor to his old friend?

Maybe ... but Chad still has to cut it on the field or else he'll be outta there.

Chad has to keep his nose clean while trying to earn his job on the regular season roster -- he was just sentenced to 1 year supervised probation after striking a plea deal in his home invasion arrest.

As we previously reported, Chad entered a random person's home after a night of boozing with the team back in October -- and was chased out of the home by a man wielding a vacuum cleaner tube.

After the Broncos cut Chad, his Hall of Fame uncle Jim met up with him to help guide along his QB workouts ... and he looked pretty good.

Kodak Black Feds Say Gun, Fingerprints, Porsche Rental Link Him to March Shooting

Federal prosecutors are laying out an overwhelming case connecting Kodak Black to a shooting earlier this year ... but the rapper's attorney says the feds are all bark with no bite.

Prosecutors say Kodak bought 3 weapons and more than 100 rounds of ammo back in February from Lou's Police Distributors -- a federal firearms licensee in Hialeah, Florida. That purchase, prosecutors allege, was facilitated after Kodak lied on a January gun application.

Kodak dropped $3,518.71 for a Mini Draco pistol, Sig MPX K 9mm pistol and Sig P238 .380 pistol.

On March 7, prosecutors say, a Porsche Panamera rented by Kodak drove to Golden Acres -- a neighborhood in Pompano Beach where he grew up -- and fired away. The feds say the intended target was a rival rapper, who was not hit but whose house -- filled with kids -- was littered with bullets. No one was hit.

Law enforcement responded to the scene and found several spent castings and the MPX K Kodak bought in February. Prosecutors say the weapon had Kodak's fingerprints and a live round in the chamber that jammed during the shooting.

Prosecutors also say cops found the Porsche abandoned 14 miles from the scene of the crime and heavily damaged. The SUV's GPS data was then downloaded ... pinpointing the SUV in the Golden Acres neighborhood at the time of the shooting.

In docs, the feds say multiple witnesses saw Kodak at the scene of the shooting. The rapper has NOT been charged with any crime and the investigation continues.

As we first reported ... federal prosecutors wanna keep the rapper behind bars and deny him bail on the heels of his arrest at Rolling Loud for allegedly lying on 2 gun applications. While laying out their argument to keep Kodak behind bars, prosecutors brought up the March 7 shooting.

Kodak's attorney, Bradford Cohen, tells TMZ ... he's yet to see any type of reports that show Kodak's fingerprints on the gun. Additionally prosecutors admitted that the weapon found on scene never fired. He adds, "Witnesses came forward to us as well and stated they did not see Kodak Black at the scene that night."

Kodak Black Too Risky for Bail ... So Say the Feds

Kodak Black is a danger to society and needs to be kept behind bars ... so say federal prosecutors who indicted him.

According to new legal docs ... the rapper's criminal history -- which typically involves firearms and drugs -- is a huge flag ... and prosecutors say he's never complied with the conditions of his probation in the past, so he'll do it again if he's released on bail.

They say Kodak's pattern of behavior is "to be arrested, get released, get arrested, get released, get arrested, get released, and so on and so on."

The docs point out that Kodak's crimes have also been "substantial and violent" ... including an armed carjacking and robbery in 2012 and possession of a weapon and fleeing police in 2016. They also tie him to a shooting in March.

Federal prosecutors argue there's no reason to believe Kodak can be trusted on the streets while he awaits trial for his latest case.

However, his attorney, Bradford Cohen, believes differently ... telling us, "If they were in such fear for the community as its being alleged, why didn't they pick him up soon after he filled out the paperwork in January? That's what this charge is about, paperwork, a non-violent crime."

As we reported ... the rapper was arrested at Rolling Loud last weekend for allegedly lying on 2 gun applications. He's also in the middle of a criminal case for allegedly sexually assaulting a teenage girl in 2016.

The judge has yet to rule on Kodak's bail.

NBA YoungBoy Back Behind Bars ... For Probation Violation After Miami Shooting

1:40 PM PT -- NBA's attorney, Jimmy Manasseh, tells us the rapper was only taken into custody because of his social media post ... and says NBA was the victim of an assassination attempt. Manasseh says NBA didn't have a weapon and didn't commit any crimes ... calling the whole thing a terrible and tragic incident.

NBA YoungBoy has been playing with fire on social media, and now -- in the wake of his Miami shooting incident -- he got burned, because cops arrested him for violating his probation.

Law enforcement sources tell TMZ ... the rapper was taken into custody immediately following a hearing in East Baton Rouge Friday, and he'll be detained for at least 30 days.

District Judge Bonnie Jackson ruled his involvement in the Miami fatal shooting last weekend, along with a social media post about it, constituted a probation violation -- even though he was the target, not the shooter.

As we reported, YoungBoy and his crew were shot at on Mother's Day outside the Trump Beach Resort near Miami, and at least one person was killed. We obtained video of the wild scene inside the hotel following the gunfire, in which the rapper rushes to the side of his GF, Kay Marie, who was hit by a bullet.

In the vid, YoungBoy's seen applying pressure to her wound as he and others try to get her to the hospital. She was eventually taken by ambulance after authorities pulled over the SUV they got in to leave the hotel.

The judge ordered the rapper detained Friday, citing the shooting and a social media post in which it appears YoungBoy talks about the shooting and threatens retaliation.

The post violates his probation from an August 2017 sentence for aggravated assault with a firearm. The judge put him on probation for 3 years after giving him a suspended 10-year prison term, and one of the conditions was that he stays off social media.

The thing is, YoungBoy's been all over social media prior to the shooting ... so, it doesn't appear he's been sticking the rules, anyway.

He's been arrested twice since then -- in Florida in 2018 and in Georgia earlier this year for weed and disorderly conduct.

We reached out to the rapper's team ... no word back so far.

MMA's Anthony 'Rumble' Johnson Pleads Not Guilty In Domestic Violence Case

Ex-UFC star Anthony "Rumble" Johnson is fighting back in his domestic violence case -- pleading not guilty to allegations he roughed up his girlfriend during a dispute earlier this month.

TMZ Sports broke the story ... the 35-year-old fighter was arrested in Palm Beach County, Florida on May 6 after his girlfriend told cops Johnson got physical with her during an argument.

According to the police report, Johnson told cops at the time he DID pick up the woman to carry her to another room because he was sick of arguing ... but insists he never intended to hurt her.

Johnson was charged with misdemeanor domestic battery -- and his attorney just submitted a not guilty plea on the fighter's behalf.

If he's convicted, Johnson faces up to 1 year in jail.

Johnson already has a domestic violence conviction on his record from 2009 -- when he pled no contest in an incident involving an ex-girlfriend.

He's due back in court for the current case next month.

Johnson -- who retired from fighting in 2017 -- was 22-6 as a pro with wins over stars like Glover Teixeira, Ryan Bader and Alexander Gustafsson.

Alec Baldwin No Punch in Parking Spot Fight ... Check the Tapes!!!

Alec Baldwin's foe in a bitter battle over a parking spot lied when he said Alec punched him ... and the actor says he has video to prove it.

In new legal docs obtained by TMZ ... Baldwin's attorneys, Luke Nikas and Alex Spiro, say there's video surveillance footage proving he didn't punch or attempt to punch Wojciech Cieszkowski during their parking squabble in November ... squarely contradicting the man's story.

Instead, Baldwin claims the footage shows both men standing face-to-face during their argument before Alec "lightly taps" the other guy's chest. Baldwin is adamant ... claiming he never threw a punch, and now says the video irrefutably backs him up.

In the docs, Baldwin also claims Cieszkowski's own medical records prove he didn't suffer any injuries ... and get this, Baldwin claims the guy's doctor recommended he take a SINGLE Tylenol and that's it.

As we reported ... Baldwin was arrested for assault after the incident, but cut a deal in January to plead guilty to a lesser charge of harassment in the 2nd degree and accept anger management classes as punishment.

But, that wasn't the end of it. Cieszkowski sued Baldwin last month for assault and slander, maintaining the actor punched, shoved and ruined his reputation by going on 'The Ellen Show' and saying he thought Cieszkowski was going to run over his wife with the car.

As for the slander allegations ... Baldwin's calling BS and says his comments to Ellen were just opinion.

Bottom line ... Alec's lawyers are asking the judge to toss the case.

Fugees rapper Pras wants to set the record straight about the feds claiming he illegally sunk millions into President Obama's coffers, or as he'd put it ... check out my new music!!!

Seriously, it really went down that way when Pras joined us Tuesday on "TMZ Live." He addressed the indictment head-on ... revealing federal agents nearly raided him during a recording session. But, when we asked point blank if he'd had any financial dealings with Malaysian financer Jho Low -- who the feds claim funneled illegal donations through Pras -- things got weird.

Pras says he plans to respond directly to the federal indictment -- but only on his upcoming EP, which he's entitled, "Elon Musk." Apparently, it's dropping next month ... as he mentioned at least half a dozen times.

He also copped to making donations to Obama, but says they were fully on the up and up. Just as importantly, Pras would say, he has an EP coming out!!!

Can't recall the name of it.

Pras has been pretty consistent with denying any nefarious activity with Jho Low, whom he acknowledges he's met, but shoots down any type of relationship.

Raz B Cops Release GF Photos ... Alleged Injuries from Domestic Assault Arrest

More details have come to light in Raz B's altercation with his girlfriend that led to his arrest ... including photos of her alleged injuries.

Cops have released photos of Raz's GF -- obtained by TMZ -- showing the aftermath of him allegedly strangling her outside a Macy's in Minneapolis. Some redness can be seen on both sides of her neck ... which the police report indicates are consistent with the neck being squeezed.

Cops also say during the arrest, Raz B stared at his GF "with intense eyes and a clinched jaw while trying to get her attention" ... which was perceived as an attempt to intimidate her.

We broke the story ... the B2K singer was arrested for domestic assault by strangulation but claimed the 2 had an argument that turned physical when she attacked him, and he was simply defending himself.

Dispatch audio and the police report reveal a witness claimed to see Raz grabbing her throat and dragging her into a parking area.

Prosecutors declined to press charges against Raz after investigating the incident, however, citing insufficient evidence. According to the report ... the girlfriend declined to speak with authorities about the case too.

The incident hasn't been the most recent bit of drama for Raz either -- he's also joined the cast of "Love & Hip Hop: Hollywood" ... and had a very short beef with Lyft.

Old news is old news!
