Valerie Fairman's Death Five Shots at Rehab in Five Years
Valerie Fairman was desperately trying to get clean for years before she died from a suspected overdose ... according to her mother.
Valerie's mom, Janice Fairman, tells TMZ her daughter checked into 5 different rehab treatment facilities over the last 5 years, but she fell off the wagon every time. She says the last time was just a few months ago ... after Valerie had served time in prison for a probation violation.
Janice tells us she last spoke with Valerie a week and a half ago, and while they didn't discuss her sobriety, Janice says Valerie sounded "way up" and "very good."
As we reported ... Valerie died Wednesday while visiting a friend Pennsylvania. Janice says she does not know the friend.

Valerie Fairman -- the 23-year-old star from the reality show "16 and Pregnant" -- has died and her family tells TMZ it appears to be an overdose.
Valerie's mom tells us she was at a friend's home in Coatesville, Pennsylvania Wednesday when the friend repeatedly called for Valerie, who was in the bathroom, but she did not come out. We're told the friend broke down the door and found her unresponsive.
The coroner is conducting toxicology tests to determine the exact cause of death but we're told it appears to be an OD.
Valerie's child, 7-year-old Nevaeh, is currently with her mom.
As for Valerie, she has been arrested a number of times. The charges include prostitution and, just last week she was busted for resisting arrest and providing false ID to law enforcement. She attempted to run from cops, who subdued her.

Sources close to the "16 & Pregnant" star tell us Jordan's spending 30 days at a rehab facility in Baltimore. Her rock bottom started earlier this month -- we're told Jordan's booze habit started affecting her job at a gym ... and she got fired.
Then late last week, after a night of heavy drinking ... she stumbled into her mother's house and fell so hard, she smacked her head on the ground and suffered a concussion.
Our sources say the injury made Jordan realize it's time to "get her life together."
Jordan's 2-year-old, Genevieve, has been living with the baby daddy's mother since last year.

"16 and Pregnant" star Lindsey Harrison just had a huge victory in court -- she's been awarded all parental rights to her 5-year-old daughter.
Lindsey, who's 21 now, got a managing conservatorship over her daughter, Aniyah Monroe Harrison-Ponce.
According to legal docs ... Aniyah's birth father, Forrest Dean Ponce, has been absent for 6 months, and failed to support his daughter for at least a year.
The judge's ruling officially terminates Ponce's relationship with Aniyah. Lindsey also gets to change her daughter's last name to her new married name ... Nicholson.

"16 and Pregnant" star Whitney Purvis says her adorable Boston Terrier Chumpy ran away and was adopted by a new family before she even had a chance to look for him, so she's fighting the rescue shelter to get the pooch back.
Purvis says Chumpy was being watched by neighbors while she was on vacation when he ran away.
Animal control says they picked up a Boston Terrier Sept. 23 and kept him for the 3-day stray hold. No one claimed him, so the dog went to Boston Terrier Rescue of East Tennessee and someone adopted him.
Whitney says BTRET refused to ante up the name of Chumpy's new fam.
BTRET claims they did everything by the book, adding the dog wasn't tagged or microchipped.
Here's where the story doesn't add up ... animal control says it picked the dog up Sept. 23, the day before Whitney left for her trip ... so someone's lying.

Former "16 and Pregnant" mom Whitney Purvis tried to clean up -- allegedly attacking her ex with a broom -- and still got herself arrested. Again.
Purvis was busted in Rome, GA on Wednesday afternoon after police say she broke into Weston Gosa's home by throwing a wheel through 2 windows -- and then whacked Gosa with a metal broom handle.
She allegedly hit Gosa in the leg and side. 23-year-old Purvis was booked for felony aggravated assault and misdemeanor trespassing.
Purvis and Gosa are familiar with jail -- they've been arrested multiple times for everything from DUIs to stealing a pregnancy test.

Weston Gosa -- the dude who knocked up Whitney Purvis on "16 & Pregnant" -- was arrested again Tuesday ... and his mug shot is the absolute best of the trilogy.
Law enforcement sources tell us ... Weston was pulled over in Georgia for speeding and driving on a suspended license. He's currently being held at the Floyd County Jail in Rome, GA on a $1,710 bond.
As we previously reported -- Weston was busted last December for smashing some electronics ... and popped again last July after allegedly crashing his car while driving under the influence (which spawned the infamous "sleeping mug shot").
Are the people who cast that show that good ... or just lucky?

'16 & Pregnant' Stars ARRESTED Alleged Computer Smashers
Whitney Purvis and her boyfriend/baby daddy Weston Gosa -- stars of MTV's "16 & Pregnant" -- were arrested in Georgia this week after allegedly smashing the hell out of some electronics ... TMZ has learned.
The Floyd County Sheriff's Dept. tells us ... 21-year-old Whitney and 23-year-old Weston were arrested on December 12th ... after someone filed a complaint accusing the two of smashing a laptop and a cell phone.
We're told a warrant was issued for their arrests ... so cops went to their home and arrested the reality stars for criminal trespassing.
Both Whitney and Weston were taken to the station where they were booked. They were each released on $1,300 bond.
It's unclear who was watching their child during the arrest.

'16 & Pregnant' Couple Regain Custody Following Massive Feces Problem
After a year-long crash course on good parenting skills, "16 & Pregnant" stars Ebony and Joshua Rendon were finally granted full custody of their three-year-old daughter again, TMZ has learned.
TMZ broke the story ... The Department of Human Services stripped the MTV stars of custody of their daughter Jocelyn last year, after police found deplorable living conditions in their home like feces covered walls, maggots and flies.
But all that's behind them now. TMZ spoke to Josh, who says he and Ebony enrolled in a rigorous program of counseling and parenting classes to learn how to care for their child and were officially granted full custody again.
Josh says, "Our family is finally back together and we are doing great. We got the help we needed and did what we needed to do. We will never make the same mistakes again.”
Josh tells us, there are no restrictions placed on the custody agreement, but over the last year they were subjected to random home inspections by the DHS.
In case you forgot, the couple is also expecting a second child and Josh tells us ... it's a girl. Mazel Tov.

"16 and Pregnant" star Jennifer Del Rio says her baby daddy is a huge threat to her family's safety, so much so, she fled 1,000 miles to get away from him and is asking a judge for sole custody of their young twins.
The MTV star recently filed legal docs in Lake County, Illinois -- where she lives, after fleeing the state of FL to escape from her ex, Joshua Smith.
In the docs, the 18-year-old says she's the primary caregiver of the couple's twin boys -- feeding them, bathing them and grooming them -- so it's in their best interest to stay with her permanently.
That's not all, Jennifer claims there's definitely NO WAY she can share custody with Joshua -- as is the current arrangement -- due to their history of domestic violence.
TMZ broke the story, Joshua was arrested in 2011 after Jennifer claimed he attacked her. Jennifer was also arrested, for a later incident, when Joshua accused her of battery ... a claim she says is bogus.
A judge has yet to rule. Calls to Joshua have not been returned.

'16 & Pregnant' Baby Daddy Popped for DUI Mug Shot's a SNOOZE
"16 & Pregnant" baby daddy Weston Gosa -- father of Whitney Purvis' kid -- was arrested for DUI in Georgia yesterday ... after allegedly crashing his car ... but the real reason we're posting this story ... dude's mug shot is legendary.
According to the Floyd County Police Department, 23-year-old Gosa smashed his car, and was placed under arrest when cops decided he had been driving under the influence. Police say Gosa was under the influence of prescription drugs at the time of his arrest.
Police also say Gosa was in possession of Xanax and Lorcet without a prescription, as well as a pipe with some kind of residue in it.
Gosa has been charged with felony possession of controlled substances, misdemeanor possession of drug-related objects, and DUI.
He's still in custody. He may or may not be aware of this.

'16 & Pregnant' Couple Pregnant AGAIN After Child Services Incident
It's barely been six months since Child Protective Services took away their daughter -- but TMZ has learned, "16 and Pregnant" couple Ebony and Joshua Rendon are PREGNANT ... AGAIN.
Joshua tells TMZ, Ebony is currently 10 weeks preggers with the couple's second child.
According to Joshua, the pregnancy was NOT planned -- but he adds, "We are extremely excited for the baby."
Here's the kicker -- CPS took away Josh and Ebony's toddler last year because of the deplorable living conditions in the Rendons' household ... including maggot-filled piles of garbage and feces on the wall. The couple was arrested for child endangerment.
Shockingly, their girl is STILL with CPS -- and we're told Josh and Ebony only get supervised visitation. The case is still pending.
But Josh tells us, he and Ebony have learned from their mistakes, completed parenting classes, and now keep their home immaculate. Josh insists ... they're 100% ready for another kid.
Josh and Ebony are currently fighting to regain full custody of their daughter.

"16 and Pregnant" star Whitney Purvis tells TMZ ... she DID swipe a pregnancy test off the shelf at a Walmart this week ... but says she's glad to report she's NOT with child.
TMZ broke the story ... the 20-year-old reality star was arrested in Georgia on Thursday when store security caught her smuggling a First Response preggo test into the women's restroom.
Purvis admits ... she did INTEND to use the test in the restroom ... but chickened out at the last minute and threw it away before she could pee on the stick.
Purvis explains, "My boyfriend was with me and he had money – but I didn’t want to freak him out. I was just being stupid with my decisions. I wish I would have handled the situation better."
The former MTV star was hauled to a nearby jail ... and released a short time later.
But before Purvis could go back to the store to legally purchase another test ... Mother Nature answered her question with a surprise visit from Aunt Flo.
Whitney's reaction -- "I was excited because I didn’t want another kid – I already have one.”
Purvis says she's learned her lesson -- noting, "I never will go back to jail or steal again in my life."

According to the police report, the 20-year-old MTV star stashed a First Response pregnancy test in a hooded jacket ... and then "took it to the restroom with her and used one."
Purvis allegedly tried to sneak out of the store through a side door ... but she was busted by store security ... who put her under citizens arrest and called local police.
According to the report, the pregnancy test was valued at $15.98.
Purvis was hauled to a nearby police station where she was booked for misdemeanor theft by shoplifting.
Purvis was featured on the MTV show in 2009. Her mother was busted for meth in Georgia last year.

"16 and Pregnant" star Whitney Purvis tells TMZ, she's had her fill of her baby daddy's slacker do-nothing lifestyle ...so she dumped the guy, and took the kid to her parents' house.
18-year-old Whitney -- who was featured on the MTV show in 2009 -- tells us, she was living with her boyfriend and their son in an apartment in Georgia up until a couple of weeks ago -- but she decided to call it quits.
According to Whitney, she ended things because her man -- Weston Gosa -- "was gone all the time. He wouldn’t come home until 3AM in the morning ... I was home with the baby and he was nowhere to be found.”
But Whitney doesn't believe Weston was cheating -- telling us, "You know when someone is cheating on you. He was just out with his friends all the time getting messed up ... I don’t want that around my kid.”
Whitney claims the couple was fighting constantly and she just couldn't take it anymore, so she moved back in with her parents -- and took their son with her.
Attempts to reach Weston were unsuccessful.

'16 & Pregnant' Couple Allowed to See Their Kid Again ... Under Supervision
The "16 & Pregnant" couple who had their daughter taken away by authorities last month -- because their house was covered in feces -- finally saw their kid again this week ... and TMZ has obtained pics of the happy 4-hour reunion.
As we previously reported, the Department of Human Services in Arkansas took away Joshua and Ebony Rendon's 2-year-old daughter Jocelyn ... after discovering the couple's deplorable feces-smeared living conditions -- and police then arrested them for child endangerment.
But the couple has since cleaned up their act -- and in return, DHS has awarded them visitation of their daughter twice a week ... for two hours at a time ... with a supervisor.
Sources close to the couple tell TMZ, E & J are serious about regaining full custody of their girl -- and to prove they mean business .. they've enrolled in parenting classes and therapy.
A court date has been set for November 10th to decide if they'll get their daughter back.
