Alec Baldwin's 'Rust' Movie Major Fines Issued Gun Safety Failures, Etc.

11:22 AM PT -- Attorney for the Hutchins' family, Brian Panish, tells TMZ, "We are pleased that the NM OSHA report has shed some light on the workplace safety issues that plagued Rust and led to the tragic and fatal events of Oct 21, 2021. Our own investigation has found overwhelming evidence of recklessness and negligence on the part of the Rust production team and others.”

10:49 AM PT -- Hannah Gutierrez-Reed, the armorer on set, issued a statement through her lawyer, Jason Bowles. Bowles said "OSHA found that she was not provided adequate time or resources to conduct her job effectively, despite her voiced concerns."

"Critically, OSHA also determined that production failed to call Hannah in to perform her armorer duties and inspect the firearm right before its use in the impromptu scene with Baldwin. As we have stated before, had anyone from Production called Hannah in back into the church before the scene to consult with her, this tragedy would have been prevented."

New Mexico's criminal authorities may not yet know who to blame for Halyna Hutchins' death -- but one state agency is laying down the hammer ... pointing to the production at large.

The Occupational Health and Safety Bureau -- which deals with this sort of thing throughout New Mexico -- issued a hefty fine of $139,793 this week after finding that Rust Movie Productions (the production company behind Alec Baldwin's flick) majorly screwed the pooch when it came to gun safety on set.

The government entity says RMP failed across the board on maintaining and enforcing industry standard protocols for gun usage in a film -- and as a result, someone died.

They point to two different misfires that had occurred during filming prior to the Hutchins incident, which the bureau says went largely unaddressed by the official safety point person among the crew ... in this case, they say that was assistant director Dave Halls.

The report notes that several crew members expressed concern after these misfires -- among other weapons-related concerns -- but it would appear no one really did anything to correct it or even address it ... via training or discipline, etc.

They also say that for the fatal shooting, Halls again broke protocol by handling the weapon and giving it to Baldwin ... without consulting a specialist during or after the gun was loaded. Speaking of that, they make a damning conclusion about live ammo -- suggesting the production heads had no system in place to make sure real bullets didn't make their way on set ... which they say is a clear violation of basic procedure for this sort of thing.

A Bureau rep says, "Management was provided with multiple opportunities to take corrective actions and chose not to do so. As a result of these failures, director Joel Souza and cinematographer Halyna Hutchins were severely injured. Halyna Hutchins succumbed to her injuries."

Again, OHSB is saying the production company as a whole seems to be responsible here -- but they go out of their way to name Halls in their remarks and findings. Their investigation was thorough ... with 1,500 staff hours dedicated to sorting through all this in the aftermath.

The criminal investigation is still underway, and of course ... lawsuits have been filed against Baldwin, the production company and others. Unclear how this report might affect things.

Dwayne Haskins' panicked wife called cops the morning of the QB's death ... saying she was concerned for her husband after he had been stranded on the highway looking for gas before he stopped responding to her, new 911 audio reveals.

In the call, obtained by TMZ Sports, Kalabrya Haskins says she was calling from Pittsburgh, PA. roughly 10 minutes after she had spoken to Dwayne on the phone in the early hours of April 9.

Kalabrya told the dispatch officer that Dwayne had called her from the side of the road near I-95 in Broward County, FL. ... claiming his car had left him stranded, and he was walking to try to find gas.

In the audio, Kalabyra can be heard saying, "He said he was going to call me back after he finished putting the gas in," but she says he never called back.

"I kept calling and kept calling," Kalabyra told the dispatch officer. "He wasn't answering. Eventually, after 10 minutes of calling the phone, eventually it cut off. It's not working now. I had his location. I just want somebody to go in the area and see if his car is there and if he's OK and if anything happened to him."

A short time later ... the officer told Kalabrya, "I don't want you to panic, but I'm going to be honest with you. We do have an incident on the highway -- but I can't confirm if that's your husband or not."

In a separate 911 call, a witness can be heard telling a dispatch officer, "There was a man hit in front of me. I was travelling on the road and I saw a dump truck hit the man."

As we reported, Dwayne was killed after he was struck by a dump truck while walking on the highway in Florida. Cops have not released further details surrounding the incident, saying an investigation is ongoing.

Dwayne was just 24 years old.

Later in the same interview, Hoda again tried getting Harry to address Will by invoking their late mother, Princess Diana, and once more ... he made it seem like he and his brother led different lives now and he very much so wanted to differentiate between himself and PW.

Josh Groban isn't ditching his mask in the air, despite the new rule allowing people to fly without covering their mouth and nose ... and he has reasons why.

A federal judge ruled Monday the CDC's mandatory mask policy on planes was illegal. The Justice Dept. has said if the CDC wants to appeal they will oblige. For now ... no masks.

U.S. Government Spend Big $$$ on Peloton Equipment

Uncle Sam seems to be all in on Peloton -- we've learned the feds have spent big on equipment ... making you wonder if Joe Biden's love of the exercise bike might be at play.

According to federal procurement docs, obtained by TMZ, the government has spent upwards of $220,000 on Peloton bikes and other exercise equipment dating back to the summer of 2020 ... before President Biden took office.

In July 2020, the Air Force placed a large order for "Peloton Bicycles/Fitness Equipment" in the amount of $33,366.

Unclear how many bikes were delivered specifically, but considering that the bikes are more than $1,000 a pop, ya gotta figure it was a bunch of them. Later in September, the Agricultural Research Service placed a Peloton bike order for another $26k worth of the bikes.

Then, 2021 rolled around -- when Biden took office -- and it seems like the feds went Peloton crazy ... ordering north of $96,000 worth of Peloton-related fitness equipment, but including a bunch of stuff from Precor Inc. -- a big name in treadmills and exercise equipment that was acquired by Peloton back in December 2020.

A rep for the Air Force tells us they have fitness centers on all of their bases, that include Pelotons and other workout equipment. They said the centers are "critical to the readiness of our airmen."

In the docs, the FBI said their Peloton equipment was for "training aids."

Among the agencies who were trying to get their sweat on ... the Air Force (again), the Army and the FBI.

2022 was quite friendly to Peloton as well from within D.C. -- it looks like the government spend upwards of $64k on more Precor stuff ... this time between the Army and Immigration.

It's no secret JB is a fan of Peloton -- he's copped to working out on one regularly ... but it became a flashpoint early in his administration as there were cybersecurity concerns from the built-in mics and cams. Officials had suggested he leave it in Delaware post-November 2020.

'Jersey Shore' Cast Films New Scenes in San Diego ... But No Ronnie

Ronnie Ortiz-Magro said he's going to return to 'Jersey Shore' for the upcoming season ... but he was nowhere to be found when his reality TV family recently started filming.

TMZ obtained these photos of some 'Jersey Shore' stars shooting scenes Easter Sunday at a winery in Southern California ... and there's no sign of Ronnie.

Here's who was at the Cordiano Winery in Escondido for filming ... Vinny, Snooki, Deena, JWoww and Mike The Situation ... plus some wives, husbands, girlfriends and boyfriends. The other main cast member who wasn't there was Pauly D.

We're told the 'Jersey Shore' cast took over the winery ... shutting the place down for filming, including the tasting room, and forcing everyone else to order food and wine from a separate bar.

Listerine Get a Cut of Mouthwash Sales ... Royalties Up for Grabs for $2.1M

Listerine antiseptic mouthwash could become someone's cash cow ... because a cut of the famous brand's global sales is up for grabs ... but it's gonna take a huge investment.

Here's the deal ... Listerine has a longstanding agreement dating back to 1881 which essentially guarantees royalty payments to a select few on a monthly basis for as long as the mouthwash product is sold.

A royalty interest in gross sales is being sold through the Royalty Exchange, with a list price of $2.1 million. So far, one bidder has offered $1 million.

The auction house says this royalty interest paid out $114,253 in the past year ... and the payments averaged roughly the same amount, $114,715, for the past 3 years ... which means it would take about 18 years to recoup a $2.1 million investment.

But, unlike royalties for songs ... where copyrights last for the lifetime of the last surviving author plus 70 years ... the Listerine royalties payout every month in perpetuity.

The unique deal is all thanks to an agreement hammered out way back in 1881 between the Listerine mouthwash inventor, Dr. JJ Lawrence, and his first distributor.

The contract includes a royalty based on the number of ounces sold, paid to him and his "heirs, executors or assigns" for as long as the product is sold ... with the ownership stake splintering between various entities over the years, including heirs, universities, hospitals, pension funds and a few individuals.

The royalties from sales in the U.S. are paid out monthly, based on the prior month's sales, while a cut from international sales is paid out on a quarterly basis ... and it's all calculated from the gross sale of Listerine Antiseptic by Johnson & Johnson.

Amir Khan is providing a blow-by-blow of the terrifying moment thieves stuck guns in his face and stole his luxury watch -- while his wife watched from just feet away -- telling TMZ Sports "it's the first time I ever had a gun literally pointed at my face."

We talked to the 35-year-old former boxing champion a day after his night out on the town with wife Faryal Makhdoom nearly turned deadly ... and asked the former boxing champion about the life-altering experience.

"In the UK, I mean the law is quite strong where hardly anyone, even the police, don't really carry guns," Amir explained, exasperated.

"So, it was quite shocking to see a gun pointed at me. And you could see that they were young. They seemed a bit nervous, and he kept saying to me, 'take off your watch.'"

We asked Amir, who had just finished eating in a restaurant about 45 minutes outside of London, if he believed the lick was somehow an inside job.

"Yeah, definitely. Obviously, I was in a restaurant taking pictures with a lot of people. I've never said no for a picture whenever someone wants to come for a picture," Khan said, explaining someone may have spotted the wristwatch and knew it was worth a pretty penny.

"Someone must've recognized that watch. I was wearing a really expensive watch which is worth like maybe $100,000. I think that news must've traveled a little bit. So, I do think that somehow it was set up. Maybe the news just went to the wrong people, and they knew I had a watch there."

We asked Khan -- who has some of the most dangerous hands in the world -- if he considered launching a counter-ambush and attempting to disarm the thieves.

Amir says the thought crossed his mind.

"I'm a fighter. There was a time I thought, he had the pistol and I can just move the pistol to the side and hit him. I know I would've hurt the guy."

But, he felt with two gunmen, and Faryal only feet away, the risk was too high.

"The only thing that stopped me was having my wife not far from me, behind me. Even if he shot the gun and even if it didn't hit me, it could've hit someone else."

After the robbery, Amir, as you'd expect, called the cops ... who immediately launched an investigation. We asked him if the police had any leads or suspects.

"The last time I spoke to them was obviously after the interview I had and they said that they saw where the car went, and they've got the car in that radius -- they've got the cameras around that car."

So far, Khan hasn't heard of any arrests that have been made.

Perhaps the saddest part about the story ... Amir, who grew up in Manchester, says he no longer feels secure in one of the world's most popular cities.

"I don't feel safe. I'm in the city London which is obviously a very well known town and everyone loves London. Now, after I was robbed, you start hearing all the stories or you start reading up on all the robberies that have happened. I mean, it's quite regularly happening," Khan says.

"I need the prime minster, the MP member of Parliament and also the Mayor of London -- what are these people doing?! They need to improve security and maybe get more police forces out or do something about this because it's not a safe place to live."
