Jeremy Jackson's threatened and attacked his girlfriend so many times, according to her -- she's finally succeeded in getting court-ordered protection from the ex "Baywatch" star.
In court docs, Elise Ginsburg says Jackson flew off the handle in July ... threatening to murder her multiple times while wielding a knife and a wine opener, strangled her and left her a bruised eye and cuts.
She's also concerned Jackson is putting together a posse to rob her house.
A judge granted a restraining order, forcing Jackson to stay at least 100 yards away from Ginsburg, her family members and her emotional support dog ... a pug named Versace.
