Autumn Crittendon of "16 & Pregnant" fame died from a combination of drugs that sadly took her life ... TMZ has learned.
According to the Virginia Department of Health, the late reality star's official cause of death was toxicity from fentanyl and para-fluorofentanyl ... an illicit synthetic opioid similar to fentanyl.
The agency lists Autumn's manner of death as accidental.

The MTV star's sister broke the news on Facebook Sunday -- penning a heartfelt message to her late sibling ... which has since been deleted. However, a source with direct knowledge has given us a bit more insight into what happened.
We're told Autumn passed away Saturday at a home she was living at with her kids, her mom and her stepdad in Henrico County, Virginia. Our sources say her mother and stepfather found her unresponsive in her bedroom, after a friend notified them that something was wrong. Paramedics then arrived and attempted to render CPR.
"16 And Pregnant" star Sean Garinger's official cause of death is blunt force injury to the head ... according to the M.E.'s office.
TMZ obtained the docs from the Raleigh, NC Medical Examiner's office, and the report on his February death says his skull was fractured in multiple places -- after his horrific freak accident involving an ATV in the driveway of his home.
As we previously reported ... Sean was driving his ATV when the ground gave way, flipping the off-road vehicle and pinning him underneath. The accident crushed his skull, tragically ending his life before help could arrive.
"16 and Pregnant" star Whitney Purvis is behind bars again ... after a judge found she failed to pony up for child support payments.
New documents obtained by TMZ show ... Whitney's arrest was ordered Tuesday by a Floyd County, GA judge who found her in contempt of court over failing to pay her ordered child support.
We've learned Whitney has since been booked and is currently in custody -- where she has been ordered to serve 48 hours in jail.

Check out these photos, obtained by TMZ, that show the patch of ground that caved in as the "16 and Pregnant" star was driving his ATV, causing the off-road vehicle to pin him underneath ... crushing his skull in the process and taking his life before help arrived.
As you can see, there's a mini-landslide on a slope of a hill next to a dirt road ... this is where Sean's mother, Mary Hobbs, says he was parking the ATV to make room for her car when the accident happened. Now, there's a makeshift memorial where Sean died ... with flowers and signs that say, "We Love You Squishy." We're told that was Sean's nickname.
Mary tells us they're planning a permanent memorial here ... which is currently in the works.
His stepdad, Dan Wolfe, is building the memorial -- and he tells TMZ ... there will be a stone wall with a laser engraving of Sean's picture, plus a custom urn made from black cherry wood, laser engraved with Sean's thumbprint, picture, birth date and death date. A garden will also surround the memorial when it's all said and done.
Mary says her son grew up riding ATVs, dirt bikes, four-wheelers, etc. ... and he wasn't doing anything reckless when he died ... it was a freak thing, and an absolute tragedy.
Of course, Sean was on Season 6 of the show with his ex, Selena Gutierrez ... and now -- their two young children will grow up without their father, something Selena is agonizing over.
Selena tells TMZ ... their two daughters, 3-year-old Dareli and 19-month-old Esmi, are too young to understand what happened to their father at the moment ... but she's glad they got to spend time with him a few weeks before he died.
TMZ also obtained some family photos and videos of his last visit with his children ... showing Sean lovingly interacting with his kids. Selena says he was "an amazing father who loved his girls" and his mom says Sean's daughters were his whole world.
Oakland A's fans won't like this ... but ex-MLB star Adrián González tells TMZ Sports he believes relocating the team to Las Vegas would be "a great move" for the franchise.
The Athletics appear to be on their way out of the Bay Area after partnering this week with Bally's Corporation to build a potential new stadium right on The Strip ... and González said in L.A. he likes the idea of it all.
The former Dodgers first baseman tells us seeing the success of the NHL's Golden Knights, the WNBA's Aces and the NFL's Raiders leads him to believe the MLB would thrive in Sin City as well.
"Vegas is doing really well with sporting teams," he said.
Of course, there's still a chance the A's could remain in Northern California ... the team is reportedly asking for around $400 million of public funding to help build the new Vegas stadium -- but if Nevada lawmakers shut that request down, the franchise could possibly explore options to stay put.

Farrah Abraham has been put on blast for allegedly taking her 13-year-old to a club ... but she says her critics don't know what's really up.
Farrah tells TMZ ... she went with her daughter, Sophia, and her friends to an "all ages" concert at The Empire Control Room in Texas Monday, thinking it would just be safer if she were there -- referencing last year's Astroworld tragedy.
Farrah's been getting slammed ever since posting about the event, with many assuming the 13-year-old was under the venue's age limit and that it was wrong to take a 13-year-old regardless.

She says there was no age limit for this particular concert. As for taking a 13-year-old, Farrah says she wanted to educate her kid on how to have a fun yet safe time at a concert.
Farrah's been working through a 12-step program for substance abuse and believes it's helped her with everything, including parenting. BTW, she says she was knockin' back H2O during the performances.

Farrah's pressing pause on her comedy ventures as she works through some legal battles -- she was charged with battery last month and pled not guilty ... this stemming from an incident in January where she allegedly slapped a security guard at a club.
There are now TWELVE women making sexual misconduct allegations against Deshaun Watson -- with 10 more accusers being vetted, according to attorney Tony Buzbee.
Buzbee represents the 7 women who have already filed lawsuits against Watson -- and at a news conference on Friday, he announced 5 more lawsuits are in the works.
"We are now representing 12 women and we will file 5 more cases in due course. We have spoken to more than 10 additional women, 22 women," Buzbee said.
To clarify, Buzbee said the additional 10 women all have similar stories to the first 12 ... but he's still gathering information before filing lawsuits on their behalf.
Of the 12 accusers, Buzbee says the women did NOT know each other before coming forward with their allegations.
The women come from different racial backgrounds -- 7 of them are Black, 2 Hispanic and 3 are Caucasian.
Additionally, Buzbee says he's been contacted by the Houston Police Dept. -- which is now looking into the allegations.
Buzbee says none of the accusers had gone to police before contacting his firm -- but says the "next step" is to submit a "package" of information to police, presumably to help launch a criminal probe.
During his news conference, Buzbee also shot down conspiracy theories that the Houston Texans were driving the lawsuits as a result of a standoff between Watson and Texans ownership. Buzbee says he lives near the family that owns the Texans but does not know them personally.
There's more ... Buzbee also notes that one of the accusers claims Watson assaulted her AFTER Buzbee had already been in contact with Watson's agent to inform him about a claim from another woman.
Buzbee says he's been personally sitting down with each accuser to vet their stories -- explaining he "wanted to make damn sure" the women were truthful.
Buzbee says the first accuser's story "certainly passes the smell test" and praised her as a brave and truthful person.
Earlier in the day, Deshaun's agent, David Mulugheta,broke his silence about the allegations against Watson -- doubling down on the notion the women are making up the accusations for "financial gain."
"Sexual assault is real. Victims should be heard, offenders prosecuted," Mulughetasaid in a statement Friday morning.
"Individuals fabricate stories in pursuit of financial gain often. Their victims should be heard, and those offenders also prosecuted."
"I simply hope we keep this same energy with the truth."
In other words, Mulughetastrongly believes the accusers are lying and he's extremely confident Watson will be vindicated in the end.
TMZ Sports has obtained all 7 of the lawsuits filed against Watson -- and they're all very similar.
Each of the women claims Watson reached out seeking a massage -- most of the women claim he first contacted them on Instagram.
But, when they met for the massage therapy sessions, the women claim Watson became aggressive in pursuit of a sex act.
One of the women claims Watson forced her to perform oral sex on him, against her will.
All of the accusers say they never consented and felt violated. They're all suing for civil assault and intentional infliction of emotional distress.
Buzbee claims he has text messages and DMs from the QB which back his client's allegations ... including an apparent apology from Deshaun after a massage session gone bad.
The first woman who filed her lawsuit against the Houston Texans QB claims Watson became aggressive during a treatment in March 2020 ... which ended after he allegedly repeatedly tried to direct the woman to massage his genitals.
In her suit, the woman claims a fully-erect Watson exposed his penis and "purposely touched [Jane Doe's] hand with the tip."
The woman claims she ended the massage immediately and began to cry. Afterward, she claims Watson sent her a text message apologizing for his behavior.
On Friday, Buzbee went public with the alleged apology text from Deshaun.
"Sorry about you feeling uncomfortable. Never were the intentions. LMK if you want to work in the future. My apologies."
For his part, Watson adamantly denies the allegations -- and claims he's the victim of a shakedown by an attorney looking to score a cash grab from his lying clients.
"Teen Mom 2" star Adam Lind's still got some growing up to do ... because he's a wanted man for all the wrong reasons.
The Minnehaha County Sheriff's Department in South Dakota tells TMZ ... there's an active warrant for Adam's arrest for allegedly failing to pay child support. The warrant was issued on Valentine's Day.
We checked with the state office that handles these matters and we're told Adam was in the hole after 2 separate judgments were handed down last September -- one for $12,365 and another for $8,757.
It's unclear which kids this is about. Adam has 2 kids (Aubree, 8, and Paislee, 4) with 2 baby mamas ... including "Teen Mom 2" star Chelsea Houska, who is now married to Cole DeBoer.
Farrah Abraham says she's been bullied and ganged up on by MTV ... and the main reason she's suing is because she doesn't want her daughter to ever be treated that way.
Farrah was in L.A. Wednesday with her daughter, Sophia -- and a couple cute pups -- and she got into the lawsuit she filed against Viacom, MTV's parent company. She says it's no shocker she's going after them, after she was booted from 'Teen Mom.'
She insists she's on to better things now, but had to take a stand with MTV to set an example for her little girl. Sophia even voiced some support for her mom.

"Teen Mom 3" star Briana DeJesus' baby daddy let a little thing like a weed citation snowball into a warrant for his arrest ... all 'cause he never showed up in court.
Law enforcement sources tell TMZ ... Devoin Austin was arrested Friday in Orange County, FL on an arrest warrant that was issued way back in August 2016.
The warrant stems from a traffic stop when cops cited Devoin for possession of weed and drug paraphernalia. He was supposed to be arraigned a month later, but never appeared -- hence, the warrant.
He could've gotten up to a year in jail or $1,000 fine for the original misdemeanor.
Farrah Abraham didn't mince words when it came to dolling out blame for Valerie Fairman's death ... and "16 and Pregnant" fell right in Farrah's crosshairs.
Farrah -- a former "16 and Pregnant" star herself -- was at Headquarter's Gentleman's Club in NYC where our photog asks her pointblank if the show and the fame played a role in Valerie's death.

First responders rushed to Valerie Fairman initially thinking she might have suffered a heart attack ... but once on scene there were signs of possible drug abuse.
TMZ got the dispatch call to the fire dept and EMS Wednesday night in Coatesville, PA ... and the call went out as a "cardiac arrest" -- but about 10 minutes later the dispatcher changes the call to a "possible DOA."
She also says paramedics are going to need containers for needles. As we've reported ... it appears the "16 and Pregnant" star died of an overdose. She'd battled drug addiction for many years.
